Saturation Pressure and Reactor Pressurizers

How does the steam bubble, in the top of a pressurized nuclear reactor pressurizer get there? Is there air in the entire coolant loop? When you fill the coolant loop with water, do you leave some space? So, you bring it up to temperature and the water expands and steam forms and, you add more water until you only have super heated liquid water under pressure and a bubble of steam? Just curious, honestly.
